To apply for a Baldwin Public Library Card, please fill out this form and click "Submit." Your Card will be mailed to you at the address provided.
Note: Only individuals who live in the Baldwin Union Free School District or own real estate property in the Baldwin Public Library Service Area (Baldwin Union Free School District in NY State) are eligible to receive a card in this manner.
If you work in or attend school in Baldwin, but do not reside in the Baldwin (NY) Union Free School District, you need to visit the Library and provide a current photo ID with home address and prove employment in Baldwin with either a paycheck or letter from the employer plus photo ID. (This card must be renewed yearly.)
